I authorize GRCC and each GRCC Representative to take, at my expense, whatever action it/he/she deems is warranted under the circumstances regarding the health or safety of my child while participating in GRCC Activities, including, providing first aid or authorizing medical personnel to provide first aid, medication, medical treatment, or surgery deemed necessary by medical personnel. I further authorize GRCC and GRCC Representatives to place my child, at my expense, in a local hospital for medical services and treatment, or if no hospital is available, in the hands of a local medical doctor for treatment. I understand that I will be solely responsible for payment of any medical, travel, and other expenses related to my child’s health, safety or medical care, and I agree to indemnify GRCC and GRCC Representatives with respect to all such expenses.
I represent and warrant to the Greater Richmond Children’s Choir “(GRCC”) that I am the parent or guardian of the chorister listed above, a minor (“my child”), and that I have the authority to give this release and waiver of liability (“Release”) on my child’s behalf, on my behalf, and on behalf of all other parents or guardians of my child. I accept this Release as an inducement to GRCC to allow my child to participate in GRCC activities. I give permission for my child to participate fully in all GRCC Activities, including but not limited to, practices, performances, travel, and other GRCC Activities. I understand that, while my child will be under the supervision of the GRCC employees, agents, volunteers, and appointees (“GRCC’s Representatives”) during his/her participation in GRCC Activities, despite GRCC’s supervision, there are intrinsic risks associated with GRCC Activities, including risks such as injury, illness, or death that may be cause by a variety of events which may or may not be foreseen or reasonably foreseeable, including, but not limited to, accident, contracting an illness, criminal activity, and acts of war or terrorism. I understand these and other risks, and I further understand that, despite reasonable supervision, GRCC cannot guarantee the health, safety, or well-being of my child while engaged in GRCC Activities. With this knowledge, on behalf of my child, myself and all other parents or guardians of my child, to the fullest extent permitted by law, I hereby waive and release any and all rights that my child, I, or any other parent or guardian of my child may have to make a claim against GRCC, GRCC’s officers and directors, and/or any GRCC Representative(s) arising from any damages, injury, illness, or death, including but not limited to Claims arising out of or resulting from travel risks, that my child may sustain or which might occur while my child is participating in GRCC Activities (“Claim”). I agree not to sue GRCC or any GRCC Representative for any and all Claims for loss of or damage to property, bodily or personal injury, loss of companionship or support, or death sustained by me or my child arising out of or relating to my child’s participation in GRCC Activities. I further agree to indemnify and hold harmless GRCC, GRCC’s officers and directors, and GRCC Representatives from any Claims I might make or which might be made on my behalf or on behalf of my child arising from my child’s participation in GRCC Activities. Furthermore, I agree to indemnify and hold harmless GRCC, its officers and directors, and all GRCC Representatives from any claims arising out of any injury, death, loss, or damage to personal property which my child causes during his/her participation in GRCC Activities. I authorize GRCC to use, reproduce, distribute, publicly perform, publicly display and create derivative works from photographs, videos, audio recordings, and film footage (“Works”) of my child participating in rehearsals and performances without compensation to me or my child. I understand that GRCC is and shall at all times be the sole owner of all rights in all such Works throughout the world. I acknowledge that I have read this document carefully and that I fully understand and agree without reservation to its terms and conditions.
WITH MY SIGNATURE BELOW I INDICATE MY UNDERSTANDING THAT THERE ARE NO REFUNDS FOR WITHDRAWALS, MISSED REHEARSALS OR TERMINATIONS OF MEMBERSHIP IN GRCC REGARDLESS OF REASON. I FURTHER UNDERSTAND THAT STUDENTS WHO WITHDRAW REMAIN OBLIGATED FINANCIALLY FOR THE FULL TUITION.
